|Degree Requirement(s)||BSN and/or MSN from an accredited nursing program.|
|Transcripts||Two official transcripts from all undergraduate and graduate institutions attended.|
|Licensure||A valid, unrestricted RN license in state of residence and/or practice in the United States.|
|Entrance Exam||GRE scores are waived for applicants with a master’s degree from an accredited program and GPA of 3.4 or higher and for applicants with a Doctor of Nursing Practice (DNP) degree from an accredited program. For applicants who do not meet these requirements and for applicants with a bachelor’s degree in nursing applying to the BSN-PhD track, GRE scores no more than seven years old from the desired semester of entry are required.|
|Letters of Recommendation||Three letters should be from individuals knowledgeable of the applicant’s professional, intellectual, and academic abilities. Letters are preferred from current or previous supervisors, faculty and/or mentors; not from peers. Letters should include a statement about the applicant’s research potential and/or ability, signed with credentials, position and contact information, and on letterhead if possible.|
|Statement of Purpose/Goals and Area of Research Interest||A one-two page paper should describe the applicant’s goals for doctoral education and for her/his profession or career. It should include a description of the applicant’s research area of interest and identify a potential faculty member/s with whom the applicant might be able to develop a common research interest.|
|Writing Sample||A two-page position paper on a current social or health issue. The paper should be in APA format and include references as appropriate.|
|Current Vitae or Resume||Current vitae or resume that includeseducation, work experience, presentations and publications, research, professional development, membership in professional organizations, and professional and/or community activities as applicable.|

The PhD Committee only reviews completed applications. Individuals with incomplete applications will be notified. ALL admission materials must be received by the selected deadline.
Upon receipt of a completed application, applicants will be contacted for an interview with two PhD Program Faculty as needed. Interviews may be conducted in person or via telephone or webcam.
After the interview, the PhD Committee will discuss the entire application and an admission decision will be made. Applicants will be notified by email and via US-mail of the Committee’s decision.
